  1. Surprising to see a lack of comment by the blogger that so frequently discourages endoscopic assessments. Pediatric guidelines regarding H. pylori appears to often advise EGD. However, for many reasons EGD could be argued as unnecessary:

    1. Culture of biopsy for H. pylori is often unreliable with many false negatives even though the organism is demonstrated in histology.
    2. In the absence of prospective study demonstrating the cancer risk in untreated pediatric patients, the impact on adult cancers and carcinogen status, we should treat and no need for EGD.
    3. The reality of our litigious society is that we are left exposed.

    Maybe tough to be critical of our close colleagues.
